Good day!
This is to respectfully request to your office to un-enrol ________________, a Grade 12 learner under
Academic Track, HUMSS Strand with LRN _______________.
The said learner was enrolled after it was confirmed on August 3, 2020 from his parent that he will be
completing the Senior High School in our institution. However, during the distribution of self-
learning modules on October 5, 2020, there was a report from the barangay official that the learner’s
modules were not claimed by the student’s parent on the following day.
The adviser immediately called the learner's parent to ask why the modules were not claimed. It
turned out that the learner lacks interest in pursuing the school year. In fact, the learner submitted
none even before nearing the end of the First Semester which shows persistent negligence.
