to all our Non-Resident Pakistani customers. Before requesting for a Cheque Book and/or applying for the Debit Card, please note the
important information pertaining to service charges:

1. Cheque Book issuance charges will be applied as per the Bank’s latest schedule of charges which is available on the Bank’s
corporate website - www.mcb.com.pk. You can find the same in ‘Quick Links’.
2. Debit Card issuance charges will be applied as per the Bank’s latest schedule of charges which is available on the Bank’s corporate
website - www.mcb.com.pk. You can find the same in ‘Quick Links’.
3. Bank will dispatch the Cheque Book/ Debit Card through international courier service. The international courier/ shipment
charges, as levied by the courier company, will be debited from your Roshan Digital Account (RDA).

Following options are available for requesting a Cheque Book and/or Debit Card:
1. Cheque Book: PKR denominated accounts can avail a Cheque Book of 25 or 50 leaves, while Foreign Currency (FCY)
denominated accounts can avail a Cheque Book of 25 leaves.
2. Debit Card: Debit Card is currently available for PKR denominated accounts only. Details of the different variants of Debit Cards
are mentioned in the enclosed application form.

To apply, please fill the attached ‘Application Form for Cheque Book and Debit Card Issuance’ and email scanned copy to
[email protected] from your registered email address. In case you are applying for either of the two services (Cheque Book or Debit
Card), please strike out the one which you are not applying for.

You are requested to ensure availability of sufficient balance in your account for processing of your request.

A. Cheque Book Requisition

authorize the Bank to deliver the Cheque Book at my registered address. I also explicitly agree to bear the
international courier charges at actual and authorize the bank to debit my account.

I would like to request for the issuance of the above selected Debit Card from the above given categories. The expected
usage of the requested card includes E-Commerce, Online Shopping and International Transactions for which I explicitly
require an International Payments enabled Debit Card as selected from the above categories.

Fees and charges are exclusive of FED. For further information please refer to the Schedule of Charges (SOC) on
www.mcb.com.pk. The validity period of the card is 5 years from the date of issue and the existing card will be renewed
automatically. Debit Card can be activated by calling MCB Contact Center Agent at +92-42-111-000-622(MCB) from your
registered number. Interactive Voice Response (IVR) facility is not available for activation of Debit Card of RDA Accounts.

I understand that a booklet containing detailed Terms & Conditions will be delivered along with the welcome pack (having the
card itself) at my registered address or branch (only in case it is returned undelivered to the Bank). I undertake that I have
understood the card product and its usage on ATMs (local & Int.), Point of Sales (local & Int.), Internet and any other alternate
delivery channel (as allowed by the Bank on various card variants). Further, I undertake that once the card is received by me,
I will contact the Bank for card activation and that will be construed as total and absolute acceptance of the Terms &
Conditions that will be delivered to me as part of the card welcome pack. I also explicitly agree to bear the international
courier charges at actual and authorize the bank to debit my account.
