Definition
1) According to elision, nursing audit refers to assessment of the quality of clinical
nursing
2) According to Goster Welfare
a. Nursing audit is an exercise to find out whether good nursing practices are
followed
b. The audit is a means by which nurses themselves can define standards
from their point of view and describe the actual practice of nursing
3) A nursing audit is a method of evaluating care that evolve reviewing patient
records to assess the outcomes were achieved

Points to be remembered:

7) adequate orientation of a data collection is essential

Advantage of nursing audit


1) Can be used as a method of measured in all areas of nursing
2) Seven function are easily understood
3) Scoring system is fairly simple
4) Results easily understood
5) Assess the work of all those involved in recording care
6) May be useful tool as part of a quality assurance program in areas where accurate
records of care are kept

Disadvantage of nursing audit


1) Appraises the outcomes of the nursing process , so it is not so useful in areas where
the nursing process has not been implemented
2) Many of the components overlap making analysis difficult
3) Is time consuming
4) Requires a team of trained auditors
5) Deals with a large amount of information
6) Only evaluates record keeping. It only serves to improve documentation not nursing
care

Lean Six Sigma focuses on eliminating defects. In healthcare, a defect can be the
difference between life and death. Lean Six Sigma to improve patient safety by
eliminating life-threatening errors.

Lean Six Sigma uses Define-Measure-Analyze-Improve-Control (DMAIC ) – a five-step


approach to process improvement. Focus on improving the patient experience by
making sure your processes consistently deliver the desired results.

Six Sigma Requires Five Steps for Quality Improvement


1) Define: Who are the patients, and what do they want? What are the capabilities of
the process being measured? What are our objectives?
2) Measure: What will improvement look like? On what data will our efforts be
measured? Metrics used to define performance in a healthcare organization often
include service level, service cost, patient satisfaction, and clinical excellence.
3) Analyze: Collect data and analyze using proven tools.
4) Improve: Implement modifications to improve the process.
5) Control: Monitor performance to maintain improvement.

Six Sigma healthcare projects currently underway in a few locations have already begun
to reduce the number of defects in specific areas, including the nurse charting process
and patient throughput in radiology. But there are many other patient care processes
that seem to be begging for immediate attention and initiation of a Six Sigma review. It’s
conceivable and reasonable to expect that the error rates cited in the IOM report could
be reduced by process implementation in the following areas:

1) Medication administration
2) Site-marking for surgical or other procedures
3) Assignment of patient caregivers
4) Case management
5) Patient falls
6) Patient restraints
7) Emergency department triage
