Chapter 2. Warnings and Cautions

Warnings, Cautions, and Notes are used throughout this manual to identify
possible safety hazards, conditions that may result in equipment or property
damage, and important information that must be considered when performing
service procedures on the BiPAP Focus.

WARNING: A condition that could cause injury to a patient, operator or technician if

the instructions in this manual are not followed correctly.

CAUTION: A condition that could cause damage to, or shorten the service life of,
the device.

NOTE: Important information concerning the construction or operation of the

device.

2.1 Warnings • To ensure personal safety and ventilator performance, service is to

be performed only by qualified service personnel.
• To avoid injury to the patient airway, DO NOT use the BiPAP Focus
where ambient room temperature is above 95 °F (35 °C) because
outlet gas temperature may exceed 106° F (41°C).
• Use only Respironics repair/service parts. Using non-Respironics
parts can alter ventilator reliability or result in damage, and
affects the ventilator warranty. Contact Customer Service.
• High voltages are present in the AC/DC power supply, power cord,
and VGA interface PCB. To avoid electrical shock, disconnect
power before cleaning or repairing the ventilator.
• DO NOT immerse this device in liquid or allow liquid to enter the
cabinet or the filter intake.
• To avoid personal injury, always disconnect external AC and DC
power sources from the ventilator before servicing or cleaning.
• To avoid personal injury or the possibility of damage to the
ventilator, regularly inspect the power cord to ensure it is free from
defects and any obvious wear, and is properly grounded.
• Never troubleshoot while a patient is connected to the ventilator,
since normal operation is suspended.
• If the ventilator has been operating, the internal parts may be hot.
Use caution when accessing.

Chapter 2
Warnings and Cautions

• To prevent disease transmission, use personal protective

equipment when handling contaminated bacteria filters or other
patient accessories. Follow manufacturers' labeling and
institutional guidelines for disposal of contaminated accessories.
• To avoid introducing foreign matter into the ventilator, inspect and
replace the air inlet filters at the recommended intervals
• To avoid cross-contamination between the test equipment and
ventilator, always install a bacteria filter between the main outlet
and test device.
• Explosion hazard. Do not operate the ventilator in the presence of
flammable anesthetic agents.
• To avoid the risk of fire, use only Respironics-approved batteries.
All battery connectors are keyed to ensure proper connection. Do
not attempt to connect a battery incorrectly.
• When running on battery, the high flows that occur during a
patient disconnect can deplete the battery in 2 minutes. Connect
AC power immediately if the Battery Depleted alarm occurs to
avoid total loss of power.
• Use only Respironics-supplied cables with the nurse call station/
serial communications connector.
• To avoid personal injury or the possibility of damage to the
ventilator, DO NOT operate this device if it has failed any
diagnostic tests.

Chapter 2
Warnings and Cautions

2.2 Cautions • Federal law (US) restricts this device to sale by, or on the order of,
a physician.
• Failure to replace a dirty filter may cause the device to operate at
higher than normal temperatures and damage the device.
• Use only Respironics-approved filters.
• Use only Respironics-approved batteries.
• The gray reusable air inlet filter must be completely dry before
use. Never place a wet filter into the device.
• Never install more than one gray reusable air inlet filter into the
ventilator. The ventilator is designed to accommodate one gray
reusable air inlet filter and one white disposable ultra-fine filter.
• During battery charging, disconnecting AC mains power before
charging completes may result in a Battery Depleted or Missing
alarm. If a Battery Depleted or Missing alarm occurs, connect AC
power immediately to avoid total loss of power.
• To prevent the premature depletion and reduced life expectancy of
the battery, store the ventilator with AC power connected and the
ON/OFF switch in the ON (I) position. When the ventilator is
stored disconnected from AC power with the power ON/OFF switch
ON ( I ) and in standby, the battery may become depleted within
two weeks. When the ventilator is stored with the ON/OFF switch
in the OFF (O) position, the battery typically becomes depleted in
three months.
If a 343 error code is displayed, remove the battery and charge it
using the BiPAP Focus battery charging cable (P/N 1040421).
This cable provides the initial charge required so that the
ventilator can recharge a depleted battery. Refer to the battery
charge instructions supplied with the cable.
• Replace any filter that is damaged.
• Do not attempt to clean or reuse disposable air inlet filters.
• Electronic components used in this device are subject to damage
from static electricity. Perform repairs only in an antistatic,
electrostatic discharge (ESD)-protected environment.
• Avoid exposing the ventilator to extreme environmental conditions.
Allow the ventilator to return to room temperature before
operation.
• Never place liquids on or near the BiPAP Focus.

Warnings and Cautions

2.3 Notes • Refer to the BiPAP Focus Operator’s Manual for product use,
additional warnings, cautions, and notes.
• Service calibration procedures require a printer to be assigned to
the service PC: this can be an internal print location (for example,
using Adobe PDF) or a printer connected to the PC. If a print error
occurs at the end of the calibration, the calibration table is
cleared and the calibration must be repeated.

Chapter 3. Theory of Operation

The BiPAP Focus ventilator provides noninvasive breathing support for adult
patients weighing 30 kg (66 lbs.) or more.
The BiPAP Focus offers two ventilation modes:
• Continuous positive airway pressure (CPAP), which provides a single
level of positive pressure to the patient.
• Spontaneous/timed (S/T), which provides two levels of positive
pressure (one during inspiration and one during exhalation), and
delivers timed breaths if the patient does not initiate a breath.
Ventilator alarms occur when high or low pressure, apnea, patient disconnect,
low power, or loss of AC power conditions occur. The system displays a real-
time estimated delivered pressure bar graph.
The BiPAP Focus includes alarm silence and alarm pre-silence features. A fully
charged backup battery provides a minimum of 45 minutes of backup power at
default settings in case AC power is not available (for example, during
transport within the hospital).
The BiPAP Focus features Digital Auto-Trak, which the ventilator to detect and
compensate for unintentional leaks, and adjust its trigger and cycle algorithms
to maintain optimum performance.

3.1.2 Muffler
The muffler reduces the noise of air flow into the blower. This is done by
channeling the air through a baffled system lined with sound absorbing
material. The channel also provides a path for air recirculation.

Muffler

Chapter 3
Theory of Operation

3.1.3 Blower
The blower consists of a speed-controlled DC motor coupled to a single stage
Blower impeller. The motor speed is monitored and adjusted by the controller PCB
based on patient settings. A pressure tap at the blower outlet enables
monitoring and comparison with the proximal (patient) pressure.

3.1.4 Flow Valve

The flow valve is a mechanical sliding assembly driven by an electromagnetic
coil. Valve commands from the controller PCB control current (-125 to
Flow
675 mA) to the coil. Flow valve movement is proportional to the current
valve through the coil. The flow valve operates as a closed-loop system, using
feedback from the flow sensor and pressure transducers.

3.1.5 Air Flow Sensor

Air flow
The air flow sensor measures flow from the flow valve. The ventilator uses this
sensor measurement to provide closed loop control of the flow valve to interpret
patient demand and calculated leak rate.

3.1.6 Diffuser
The diffuser is located on the output of the flow valve and is designed to
provide a reduction of air turbulence.

3.2 Electronic System The BiPAP Focus electronic system controls, monitors, and displays patient
and ventilator data. Two microcontrollers (on the user interface and controller
PCBs) interpret user and sensor inputs while controlling inspiratory and
expiratory pressures according to the selected ventilator settings. Figure 3-2

Maintenance a battery incorrectly.

NOTE: Recharge the battery if a Low Battery or Battery Depleted alarm occurs
during a pre-operational check.

NOTE: If a Batt. Charge Failure alarm occurs or the battery does not begin to
charge after 1 hours, the battery may be fully depleted: see section 4.4.4.

NOTE: Battery alarms sound near the end of the battery test. Gas delivery
continues until the battery is depleted.

Test the battery once per month. A fully charged battery is designed to support
ventilation for 45 minutes at default settings, and should be replaced if it
cannot.

4.4.1 Battery Operating Time

The length of time the ventilator can operate on internal battery power
depends primarily on ventilator CPAP/IPAP settings (Table 4-2).

Table 4-2: Battery Operating Time

Approximate Battery
CPAP or IPAP Setting
Operating Time

4.4.2 Battery Charging

The nickel metal hydride (NiMH) internal battery automatically charges when
the ventilator is connected to AC power only if the power switch is in the ON (I)
position. The ON AC Power LED lights to indicate that AC power is connected
and the Battery Charging LED lights to indicate that the charging cycle is in
progress. The battery is fully charged when the Battery Charging LED turns off.

The charging cycle operates in Normal and Standby mode. Battery charge time
is typically less than 5 hours, but can be longer depending on ambient
temperature.

It is important to maintain a fully charged internal battery in case of a power

outage. Recharge the battery if the ventilator has been in storage. An unused,
fully-charged battery depletes within 3 months if the power switch is OFF (O),
and within 2 weeks if the power switch is ON (I).

CAUTION: During battery charging, disconnecting AC mains power before charging

completes may result in a Battery Depleted or Missing alarm. If a Battery
Depleted or Missing alarm occurs, connect AC power immediately to
avoid total loss of power.

5.10 Significant The BiPAP Focus Ventilator generates a log of system events that can be used
for troubleshooting, including settings changes, alarms, and codes. If a
Event Codes ventilator inoperative condition or unexpected reset occurs, the significant
event log provides a record of any ventilator setting changes, alarm conditions,
or error conditions that were in effect at the time.

The significant event log records up to 200 of the most recent events, with the
most recent events listed first. The ventilator maintains the significant event
log regardless of whether there is power to the ventilator.

Each significant event log entry includes the following:

1. Event ID: A code and brief text description of the event.

2. Date: Each event is date-stamped in a month/day/year format (for

example, 10/20/02).

3. Time: Each event is time-stamped in hour/minute format (for example

09:15).

4. Previous/current value: For settings changes, the previous and current

values are displayed.

Use the arrow keys to scroll through the log. Table 5-3 lists event IDs and
corrective actions.

NOTE: When performing corrective actions:

• Perform the corrective actions in the order listed.
• Always retrieve and save the DRPT if possible before replacing
parts. If the corrective action suggests swapping the user
interface or controller PCB, retrieve the DRPT before installing
the replacement PCB.
• To verify that replacing a part corrects the problem,
temporarily install (swap) a replacement part. If the
replacement corrects the problem, verify that the problem
recurs with the original part before replacing it.

service PC: this can be an internal print location (for example, using
Adobe PDF) or a printer connected to the PC. If a print error occurs at the
end of the calibration, the calibration table is cleared and the calibration
must be repeated.

7.7 Electrical Safety The electrical safety test verifies that the forward and reverse leakage current
are within specified limits.
Test
Required equipment:
• Electrical safety analyzer

NOTE: During this test the neutral switch should be closed, not open.
IAW IEC 60601-1 test procedures.
Follow these steps:

1. To connect to ventilator ground, connect the electrical safety analyzer

to serial port shell on the ventilator back panel (Figure 7-26).

Figure 7-26: Serial Communications Port

1. Attach the test orifice to the gas outlet port, and turn the ventilator on
and to begin gas delivery.

2. Check that forward and reverse leakage current is:

< 300 uA for ventilators connected to 100-120 VAC
< 500 uA for ventilators connected to 220-240 VAC.

3. The electrical safety test is complete.

NOTE: Ground impedance testing (25 A for US and Europe, 40 A for Canada) is
not applicable to the BiPAP Focus ventilator system. The earth
connection from the rear panel of the ventilator is a functional earth for
EMC purposes only. The Protective Earth system is within the power
supply unit as it has no conductive exposed parts. The accessible
functional earth is allowed by IEC 60601-1 because the BiPAP Focus
meets enclosure leakage and separation requirements.
It should be noted that the ground impedance test per IEC 601 is a type
test and not a production line or routine safety test. Respironics
recommends against testing Ground Impedance since it would require
opening the power supply. The functional earth from the ventilator should
be checked by continuity testing (ohmmeter or battery/buzzer) to assure
the ground is intact. The shield of the power supply output cable provides
the EMC ground connection to the protective earth conductor inside of
the power supply unit.

and press Enter to test the alarm.

6. Select Backup Audible On and press Enter to test the alarm.

Figure 7-27: Diagnostic Mode: System Test Screen

7. Set the DMM to measure resistance:

• Connect the DMM to pins 1 and 6 and verify that resistance is
infinite (open circuit) when Remote Alarm Off is selected on the
System Test screen.
• When Remote Alarm On is selected, verify that the DMM
measures 0 ± 3  (closed circuit).
• Connect the DMM to pins 1 and 9 and verify that resistance is
infinite (open circuit) when Remote Alarm On is selected.
• When Remote Alarm Off is selected, verify that the DMM
measures 0 ± 3  (closed circuit).

8. Remove the DMM leads and return to the System Test screen.

9. Select Alarm LED On, then Silence LED On, and then On Battery LED

7.9 Pressure The pressure accuracy test verifies that pressures are within specified limits.
Accuracy Test 1. Attach an oxygen enrichment adapter to the ventilator outlet port and
occlude.

2. Set the pneumatic analyzer pressure range to measure at least 20

cmH2O, and connect the analyzer to the oxygen enrichment adapter.

3. If the ventilator is not already in normal ventilation mode, cycle power

to the ventilator to enter normal ventilation mode.

4. Select CPAP mode and set the pressure to 4 cmH2O. Verify the
pressure reading and record the results.

5. Set the pressure to 20 cmH2O. Verify pressure reading and record the
results.

6. Select: S/T mode, IPAP = 10 cmH2O, EPAP = 5 cmH2O, Rate = 10

BPM, and I-Time = 2.0.

7. Verify that the BiPAP Focus switches between IPAP and EPAP. Record
the results.

8. Activate the ramp time setting and set ramp time to 5 minutes.

9. Observe the display and the analyzer reading. Verify that the pressure
increases to the target pressure in 5 minutes and record the results on
the data form.

Chapter 8. Component Removal/Installation

This chapter describes how to remove and install replaceable ventilator

components.

WARNING: To avoid personal injury, always disconnect external AC and DC power

sources from the ventilator before servicing or cleaning.

CAUTION: Electronic components used in this device are subject to damage from
static electricity. Perform repairs only in an antistatic, electrostatic
discharge (ESD)-protected environment.

NOTE: Always retrieve and save the DRPT if possible before replacing parts.
Removing the user interface PCB deletes the serial number, significant
event log entries, and ventilator preference settings. Review the
significant event log before removing the user interface PCB.

NOTE: To avoid damaging the controller PCB pressure transducers, carefully

remove tubing to avoid pulling on the sensors.

10.2 Patient Circuit The BiPAP Focus System automatically compensates for pressure drops
associated with a Vision single patient use circuit and the mandatory bacteria
Details filter. The pressure drop increases as restrictive elements (for example,
humidifiers) are added to the circuit. Always use a manometer to verify patient
mask pressure.

RF 150kHz to equipment should be used no closer to
IEC 61000- 80MHz any part of the BiPAP Focus System,
4-6 including cables, than the recommended
separation distance calculated from the
Radiated RF 3 V/m 3 V/m equation appropriate for the frequency of
IEC 61000- 80mHz to the transmitter.
4-3 2.5GHz
Recommended separation distance:

d = 1.2 P

d = 1.2 P 80MHz to 800MHz

d = 2.3 P 800MHz to 2.5GHz
where P is the maximum output power
rating of the transmitter in watts (W)
according to the transmitter
manufacturer and d is the recommended
separation distance in meters (m).
Field strengths from fixed RF
transmitters, as determined by an
electromagnetic site surveya, should be
less than the compliance level in each
frequency rangeb.
Interference may occur in the
vicinity of equipment marked
with the following symbol:

a. Field strengths from fixed transmitters, such as base stations for radio (cellular/
cordless) telephones and land mobile radios, amateur radio, AM and FM radio
broadcast, and TV broadcast cannot be predicted theoretically with accuracy.
To assess the electromagnetic environment due to fixed RF transmitters, an
electromagnetic site survey should be considered. If the measured field
strength in the location in which the BiPAP Focus System is used exceeds the
applicable RF compliance level above, the BiPAP Focus System should be
observed to verify normal operation. If abnormal performance is observed, addi-
tional measures may be necessary, such as reorienting or relocating the BiPAP
Focus System.
b. Over the frequency range 150kHz to 80MHz, field strengths should be less
than 3 V/m.
Note: These guidelines may not apply in all situations. Electromagnetic propagation is
affected by absorption and reflection from structures, objects, and people.
