Generic name: Omeprazole
Brand name: Prilosec, Prilosec OTC, Zegerid, Zegerid OTC
Classification: Proton-Pump Inhibitor
Action: Omeprazole inhibits proton pump activity by binding to
hydrogen-potassium adenosine triposphatase, located at
secretory surface of gastric parietal cells, to suppress gastric acid
secretion.
Indication: Omeprazole is use to treat symptomatic GERD
without esophageal lesions, Erosive esophagitis. Pathologic
hypersecretory conditions (Zollinger-Ellison syndrome), and
Helicobacter pylori infection. This is also given for a short-term
treatment of active benign gastric ulcer, duodenal ulcer, and
frequent heartburn.
Dosage: 40 mg IVTT 1hr PTOR and 40mg/cap 1 cap PO at
bedtime PTOR
Side Effect: Common side effects of omeprazole are headache,
abdominal pain, diarrhea, nausea, vomiting. flatulence,
dizziness, upper respiratory infection, acid reflux, constipation,
rash and cough
Adverse Effects: Omeprazole adverse effects in CNS are
asthenia, dizziness,and headache. In GI are abdominal pain,
constipation, diarrhea, flatulence, nausea, vomiting, and acid
regurgitation. In Musculoskeletal is back pain. For respiratory
are cough and URI. And skin rash.
Contraindication: Contraindicated in patients hypersensitive to
drug or its components and in patients receiving rilpivirine-
containing products.
Nursing Responsibility:
Evaluate for CDAD in patient who develop diarrhea that
doesn’t improve.
Assess patient for signs and symptoms of cyanocobalamin
deficiency (weakness, heart palpitation, dyspnea, parasthesia,
pale skin, smooth tongue, CNS changes, and loss of
appetite).
Monitor patient for signs and symptoms of acute interstitial
nephritis. Discontinue drug if signs and symptoms of
cutaneous lupus erythematosus SLE develop; refer patient to
the appropriate specialist for evaluation.
Monitor magnesium levels before starting treatment and
periodically thereafter.
Monitor patient for signs and symptoms of low magnesium
level, such as abnormal HR or rhythm, palpitations, muscle
spasm, tremors, or seizures.
Health teaching:
Tell patient to swallow tablets whole and not to open, crush,
or chew them.
Give patient instruction on how to take oral suspension.
Instruct patient to take drug at least 30 to 60 minutes before
meal.
Caution patient to avoid hazardous activities if dizziness
occurs.
Advise patient that Prilosec OTC isn’t intended to treat
infrequent heartburn (one episode of heartburn a week or
less), or for those who want immediate relief of heartburn.
Inform patient that Prilosec OTC may take 1 to 4 days for
full effect, although some patients may get complete relief of
symptoms within 24 hours.
Teach patient to recognize and report signs and symptoms of
low magnesium levels.
