REMEMBER:
We are oftentimes asked to fill out forms whenever we use documents at the bank, school, or
office. This is essential that we cannot avoid doing. For these reasons, we need to master how
to fill out different forms accurately.
1. Deposit Slip
is a small paper form that a person includes with a bank deposit to show how much money
he/she is putting in a bank account.
It contains the deposit date, name, and bank account of the depositor, and the amount to be
deposited in the form of checks or cash, and the signature of the depositor.
Specifically, a person who deposits or places money in a bank account is called a
DEPOSITOR.
How to Fill out a Deposit Slip?
1. First, write your name and bank account number.
2. The, supply additional details, such as transaction date, and any branch information required.
3. Lish the cash amount breakdown of your deposit and the total amount therafter.
4. Affix your signature over printed name found on the lower part of the form.

2. Withdrawal Slip
Is a small paper form which has to be filled out before making a withdrawal of money from a
bank.
After filling out the bank slip, a person signs on it in the presence of a bank teller.
A BANK TELLER is an employee of the bank whose job is to help bank customers with their
banking needs, such as depositing checks or cash on making a withdrawal.

to collect and update information from parents of current and future students (including users
with "parental access" rights), and the students themselves.

Activity 1: Read and understand the sentences below and do the following. Underline the
compound word found in each sentence, and looks for the meaning of it in the choices given.

Activity 2: Figure out the meaning of the italicized compound word by looking for its

Are words formed by putting two or more words together to form a new word with a new
meaning
To understand the meaning of the compound, it is important to determine the meaning of each
word that makes up the new word.
Example:
Compound words may be closed (written as one), open (written with space between words), or
hyphenated (connected by a hyphen). Study the table below.

Open Time zone, home port, mother tongue, hot dogs
Hyphenated Mother-of-pearl, son-in-law, passers-by, name-calling
The meaning of the compound words can be inferred through context clues with the use of:
1. Synonyms- one of the two or more words of the same language that have the same meaning
2. Antonyms- are words of opposite meaning.
Inference clue is a kind of context clue. In an inference clue, some kind of information is given
in the sentence to help you figure out the meaning of unfamiliar word.
