Form 3. LAC Session Report

This form is used by the LAC Facilitator to summarize each LAC (Learning Action Cell) session. It collects information such as the LAC ID, date and time of the session, number of members present, materials used, and the facilitator's evaluations of member participation and their own facilitation. The facilitator rates various statements related to preparation, discussion, engagement, and feedback on a scale from Strongly Disagree to Strongly Agree. It also collects comments on what went well, any problems encountered and how they were addressed, additional topics discussed, and plans for the next session.
