OATH OF UNDERTAKING

I, ___________________________________, of legal age, married/single and with postal


address at ________________________________________, states that:

1. I am honored to be hired as a
____________________________________________________of the University and was given
a regular teaching load of _____ units and/or, honorarium load of _____ units as reflected in my
teacher’s program this __________ Sem, A,Y, _____________;

2. By accepting the teaching appointment in the University, I commit myself to observe and
practice the principles that underlie the vision, mission and the policies of the university,
specifically in the:

a. submission and implementation of the Outcomes Based Teaching and Learning Plan;
b. encoding of accurate grades on time;
c. observance of the required teaching hours and following the Class Schedule;
d. regular attendance in meetings, seminars, trainings and other official activities;

3. I am willing to accept other designation and perform additional responsibilities and functions
other than teaching, within the duration and terms of my assignment/s.

4. I will give priority to my teaching load in the University of Makati and will only teach in other
school or work in other company if a written notice is submitted and permission is granted and
assure that the said work will not be in conflict nor affect my efficiency in the delivery of my
teaching assignment.

If in case there are information and discoveries that my work in other school/company
collapses/encroaches with my official time, this will be a cause for an appropriate administrative
case against me.

5. In case I will not be able to finish the semester or term of my course load due to unavoidable
circumstances or for any valid reason, a one month ahead written notice shall be submitted to the
College Dean/Executive Director with the necessary accomplished clearance and duly approved
by the Management Committee (MANCOM).

6. I will honestly commit myself to protect and secure the confidentiality of any information,
systems, data and materials of the university.

7. I am bound to submit on time all the employment requirements of the Human Resource
Management Office (HRMO) of the University;

8. By affixing my signature in the Teacher’s Program, I undertake to abide with my commitment


for the duration of my employment. The Teacher’s Program serves as an explicit contract that I
am obligated to fulfill my responsibilities as a
________________________________________of the University. As such, by accepting this
position I assume a contractual obligation to live up to the terms and conditions of the contract
and my oath of office.

The University shall have all the right and full authority to pre-terminate my appointment, take legal
action or withhold any compensation or other benefits for my non-compliance or violation of any of the
aforementioned terms and conditions.
