1. This Oflice announces the conduct of evaluation of documents for credit


points and interview of qualified applicants for Master Teacher I and II positions
for SY 2O2O-2O21 by the Personnel Selection Board {PSBI.

2. All interested qualified applicants should submit the following pertinent


documents (original and photocopy) to the Schoo1 Head on or before October 12,
2O2O and subsequently indorsed to the Human Resource Section for
authentication, preliminary screening and evaluation qn o! before October 16,
202o:
1. Letter of Intent (addressed to the Schools Division Superintendent)
2. Personal Data Sheet (CSC Form 212 revised2OLT. (Can be downloaded
at www.csc.gov.ph)
3. Transcript of Records/ with Special Order for Private University
4. Latest Approved Appointment as T-III (for MT-I applicant) and as MT - I
(for MT-II applicant)
5. Updated Service Record
6. Certificate of Registration and License from PRC
7. Performance Rating for the last three (3) consecutive years signed by the
principal (at lea.st 33 points) for MT-I and (at least 35 points as Master
Teacher I or ESP I for MT-II
8. Certifrcate of Trainings Attended as indicator of leadership potential and
Accomplishments/Reler to the attached Checklist of Basic Requirements for
Master Teacher I and II).

3. All documents shall be arranged according to the checklist and fastened in


a long folder. Do not use pocket folders / binders. Documents under item number
8 shall be within the last five years and after the last appointment. Likewise, only
seminars and trainings conducted by or approved by tfre concerned DepEd I-evel of
Governance.

4- Failure to submit the required pertinent documents on the prescribed


period of submission will no longer be accepted.

5. Schools shall reconstitute ttreir School Selection Committee composed


of the principal as chairman and four members preferably master teachers and
or head teachers and representative of teachers club. In case the school has no
master / head teachers, SSC members shall be selected based on competence
and integrity.

The SSC shall a) assist the applicants to orgatizr their documents; b)


conduct initial screening of documents of applicants using the attached Score
Sheet; and c) indorse applicants' documents to the Division Office, Attn: Ms.
Elfie B. Montiano, HRMO, Human Resource Section.
