AFFIDAVIT OF LOSS

I, CRISTINA MUGA, of legal age, Filipino, married and a resident of Anislag,


Corella, Bohol, Philippines, after having been sworn in accordance with law, depose
and say:

That I am a holder of an account No. 010-20-1090977with the Katipunan


Bank of Bohol, Palma St., Tagbilaran City, Bohol;

That as such, I was issued one (1) Savings Passbooks by the said bank;

That said Katipunan Savings Passbooks were in my possession but the same
were lost / misplaced in an event and place which I could no longer recall;

That I exerted diligent efforts to find and recover the aforesaid Katipunan
Savings Passbooks, however, my efforts proved futile as until now, their
whereabouts still could not be ascertained, hence, I considered them to be lost for all
intents and purposes;

That the said Katipunan Savings Passbooks were not transferred to any
person for consideration or used as collateral to secure obligations;

That I execute this affidavit voluntarily to attest to the truth of the foregoing
statements to support my request with Katipunan Bank for the issuance of another
passbooks in lieu of the lost one;

That I hereby hold free and harmless the said bank for the issuance of
another passbook in my favor and I undertake to surrender with the bank the
aforesaid lost Katipunan Savings Passbooks in case it is found later on;

That further, I execute this affidavit for whatever legal purpose this may serve.
