ADDENDUM TO MEMORANDUM OF AGREEMENT
ROMAN CATHOLIC ARCHBISHOP OF PALO, INC.
KNOW ALL MEN BY THESE PRESENTS:
This Addendum to the Memorandum of Agreement (the “ADDENDUM”) is
made and entered into by and between:
The DEPARTMENT OF PUBLIC WORKS AND HIGHWAYS (DPWH), an
agency of the National Government with principal office address at Bonifacio
MOST REV. JOHN F. DU
Drive Port Area, Manila, duly represented herein by Regional Director NERIE D.
BUENO, DPWH-Region VIII, hereinafter referred to as “DPWH”;
-and-
SCHISTOSOMIASIS HOSPITAL (SH) a government entity organized and
existing in accordance with the laws of the Republic of the Philippines, with
principal office at Schistosomiasis Hospital, Palo, Leyte, duly represented herein
by LYN L. VERONA, MD, MHA, hereinafter referred to as “SH”;
-and-
ROMAN CATHOLIC ARCHBISHOP OF PALO, INC. (RCAP), a
corporation sole organized and existing under Philippine laws with principal office
at the Archdiocesan Chancery, Brgy. Salvacion, Palo, Leyte, represented herein
by the Archbishop of Palo, MOST REV. JOHN FORROSUELO DU, hereinafter

referred to as “RCAP”;
W I T N E S S E T H:

WHEREAS, the PARTIES hereto entered into a Memorandum of
Agreement (“ORIGINAL MOA”), said MOA is hereto attached as Annex
“A”, involving a DPWH project providing shelters for front liners and other
healthcare workers of SH through the “We Heal as One Offsite Dormitory
for Medical Personnel” which project shall be established on a property
owned by RCAP;
WHEREAS, during the subsistence and effectivity of the Original MOA,
PARTIES have expressed intentions which were not made part of the Original
MOA;
WHEREAS, these after-signed intentions by the PARTIES are necessary
and significant stipulations which must be incorporated in writing;
NOW THEREFORE, for and in consideration of the foregoing premises
and in addition to the terms and conditions set forth in the Original MOA, the

parties herein agree as follows;
A. Responsibilities of the SH the following:

10. Issue a Certificate of Acceptance after the project is verified as
having no defects;
11. Submit to the DWH a Monthly Utilization Report of the offsite
dormitory not later than the 10 th day of the succeeding month, using
the attached format provided by the DPWH.

B. Upon termination of the Original MOA, all improvements introduced on the
property shall redound to the benefits of RCAP by way of Donation without
need of executing a deed of donation to that effect.
C. That all whereas clauses, terms and conditions set forth in the
ORIGINAL MOA inconsistent with this ADDENDUM shall be rendered
inefficacious and invalid while all other provisions continue to be in full
force and effect in accordance with its terms.
