This is to certify that the existing structures with the following description below had
been inspected and evaluated by the undersigned and found to be structurally sound,
safe and stable and fit for the use of occupancy as stated.

Type of Structure : As-Built Three Storey with Roof Deck Office Building

Brgy. 658, zone 70, Metro Manila

OWNER : Aquila Resources & Holding’s Inc.

This certificate has been issued upon the request of the building owner to attest the
safety of the building structures and for whatever purpose it may serve.

This certification is considered void if there is alteration, such as construction of


additional units, rooms, floors, and renovation without proper permits and prior to
approval of the undersigned.

In any occurrence of seismic, this certification will lose validity unless re-approved by
the undersigned.
