Note: Fasten requirements in a long brown folder

1. Sworn Application Form


-Notarized Application form in 2 original copies

2. Proof of Income (Photocopy of Any of the following)


-Income Tax Return (ITR)
-Certificate of Employment and Compensation

3. Marriage Certificate / Birth Certificate (Photocopy)

4. Birth Certificate of Children (Photocopy)

5. Family Picture

6. Photocopy of AFP/PNP ID

7. Certificate Present Assignment or Duty Status (Original)

8. Photocopy of two (2) consecutive latest pay slip

9. Photocopy of latest notarized SALN

10. 1pc 2x2 ID picture (attach on application form)
