Appointment Booking Form

You can complete this Nationality Checking Service Appointment form in Adobe Acrobat and print
it out, or print it and complete it by hand, using block capital letters. Return the form to Nationality
Checking Service, Council Headquarters, Almada Street, Hamilton, ML3 0AA or for a quicker
response email the attachment to [email protected] (you will need Acrobat Reader
version 8 or later to be able to save the completed form. Go to www.adobe.com to download a free
copy).

Before applying for an appointment please check you have all relevant documents. If you attend
the appointment without the relevant documents you may have to make a further appointment at
an additional cost. Full details can be found on the UK Border Agency website
www.ukba.homeoffice.gov.uk .

A separate appointment booking form is required for each family member applying.

The fees for the Nationality Checking Service are at the end of this document. When we receive
your completed booking form we will phone you to confirm the date and time of the appointment
and we will collect the fee for your appointment by debit/credit card over the phone. The fee must
be paid when making the appointment.

You must make sure that you have fully completed the most up to date and valid application
form before attending the Checking Service appointment.

The following is a list of the most common types of applications:
This list does not cover all the application types which can be submitted using the
Nationality Checking Service (for further information please contact the Nationality
Checking Service team on 01555 673133 or e-mail [email protected])

AN1 Single adult 6(1)  Passport to cover the five year qualifying period
 Confirmation of immigration status

Without the relevant documents we cannot offer you an appointment. Any documents not in
English must be submitted along with an official translation.

*Alternative evidence examples:- letters from employers/schools/colleges/universities confirming


dates of work/attendance, P60’s, wage slips (must be a complete set of either 52 weekly, 26
fortnightly or 12 monthly wage slips), National Insurance contributions letter, letters from Inland
Revenue confirming payment of taxes (if self employed), letters from DSS confirming receipt of
unemployment benefit, income support or other benefits including child benefit. Utility bills are not
accepted as alternative evidence.

If alternative evidence is being submitted please give details at question 15.

All persons aged 18 or over must attend the appointment in person.

(The qualifying period is either three or five years, for example, if you are submitting a 6(2)

appointment in 2013, please list your absences from the United Kingdom during 2010. If you are
submitting an Adult 6(1) application and are completing this form for an appointment in 2013
please list your absences from the United Kingdom during 2008)

This fee is non-refundable. Payment can be made by debit/credit card. This fee is payable for the
checking of your application(s), Home Office application fees also apply.
