conduct a pilot testing for their research study entitled Satisfaction of Grade 12 Senior
High School Students towards School’s Health Services.
This study examines learner’s level of satisfaction with the health appraisal program of
a learning institution. It is hoped that the results of this study would be able to add up
to the body of knowledge concerning the satisfaction rate of health appraisal, thus can
be able to spread awareness for the benefit of the community. The researchers are
asking for your permission to excuse the following students, from your class, as listed
below. The survey will only take up a maximum time duration of 15 minutes in
answering the said research questionnaires.
