Parsys Cloud is a cloud-based solution allowing a secure and user-friendly exchange of


medical data and video-communication between remote sites.

It allows an isolated crewmember on a ship to connect with a remote medical center for
relief or a health professional to receive the support of an expert on a specific issue.

Parsys Cloud is a technical integration brick at the server level, in charge of insuring the integrity of data
transmitted from clients’ workstations and technical interfaces with the Electronic Health Record (EHR).
Opening a Cloud account is a necessary prerequisite for any entity wishing to acquire biomedical sensors.

Network Infrastructure
Parsys Télémédecine is particularly vigilant to data flows transiting in both directions between the “Pivots”
(institutions) and dozens of other institutions gravitating around it, each with their own security policy and
network topology.

Based on open and modern standards such as Websocket and WebRTC, the secure transport layer is
designed to switch seamlessly NAT routers, firewalls, and proxies from an HTTPS elevation. In nominal
configuration, sending data requires no additional port opening for concerned institutions’ IT services.

Following the latest industry conventions, Parsys Cloud has a Web service communication layer allowing a
perfect and modular integration thanks to an REST API (HTTPS / JSON).

Scalability and Flexibility


Parsys Cloud is conceptually agnostic to the type of medical data transiting through its mediation network.
Compatible with biomedical sensors provided by Parsys Telemedicine, it is also designed to easily
accommodate other sensors that may be integrated in future evolutions. The existence of a local software
and a cloud server allows any potential interfacing. The interfacing can be done either through external
"Clouds" – as required by many large public welfare manufacturers through Web services and the
authentication OAuth2 directly from the hub – or on the client’s workstation in order to communicate with
drivers and exclusively local addressable interfaces. Mobile solutions (SDK iOS / Android) allow remote
monitoring such as home monitoring.

ParsysRTC

ParsysRTC (for Real Time Communications) is the newest-


generation of fully web-based videoconferencing solution for
telemedicine.

Its main advantages over traditional systems are:


• A straightforward 100% HTML5 / JavaScript integration SDK.
• Easy extension of the signaling stack with new real time
events (subscribe to updates, alerts, etc.).
• Seamless integration with medical data (stethoscope,

Integration and ease of use

ParsysRTC is the only videoconferencing solution exclusively


Calling an expert

appropriate ergonomics.
Unlike competing enterprise videoconference solutions, which
are highly monolithic, ParsysRTC is built from the ground up to be
modular, and easily integrated within an existing web platform.

Manageability

RTC supports authentication and authorization of users through


a simple web interface accessible to administrators to create
and edit user accounts. Since RTC is not a phone platform such
as old VoIP-derived products, the management interface can
be extremely trivial to use. At a minimum, users only need an ID Full-screen video
(we suggest using email addresses) and a password. Up to
several million accounts can be created, using MongoDB built-in
the signaling server as a persistent backend.

Deployment flexibility
We understand that the technical environment of each client is unique. This is why ParsysRTC is designed in a
modular and adaptable way, with many configuration options or possible further developments.

The architecture includes two types of servers:


• ParsysRTC Video Signaling Server, which controls the connection of one or more communications;
• ParsysRTC Video Relay Server, which enables the video stream when a point-to- point connection is not
possible, interconnects with external systems if necessary, or allows voice recording or video if required
by medical procedures.

ParsysRTC may also allow communication through firewalls (or NATs) through international standards (ICE,
STUN and TURN).

The video platform can either be hosted by the client or by Parsys. In the latter case, it is hosted on a
medically secure architecture, in conformance with HIPAA, medical CE and EU privacy guidelines, including
FIPS-approved best practices and certified 128bit EAS encryption.

The centralized architecture allows remote configuration and update - automatic or manual - of all stations.

A solid technological base
ParsysRTC is largely based on an implementation of the WebRTC standard. Originally developed
by Google since 2011, WebRTC is now included natively in Google Chrome and Mozilla Firefox (MS
Edge and Apple Safari have announced support in the near future.

Consequently, the code is constantly tested, qualified and audited by a large community of
experts that help our developers. This gives us a major technological advantage over traditional
proprietary solutions, especially in terms of reliability and performance.

In addition, WebRTC is now ETF and W3C-compliant (telecom international standards), which
guarantees a continuity and compatibility with international standards in the present and future.
For example, the interconnection between WebRTC and SIP existing equipment (Cisco, Microsoft
Lync, etc.) is possible.

Flow Security (TLS RSTP, AES, digital certificates) is also in line with the state of the art on the market.

An unmatched video and audio quality


The video codec used by ParsysRTC is VP9. Finalized in early
2013 by Google and Vydio, VP9 is the latest generation in
optimizing the signal and thus uses up to 50% less
bandwidth than competing products currently available
(Cisco, Skype, VSee, Digigone, Lync, etc.) using H264 or
VP8.

The audio quality is a topic that is rarely addressed by
suppliers of conventional videoconferencing. We, however,
believe that the quality of the audio, and the reduction of
bandwidth caused by optimal compression, are key
considerations for a medical consultation.

Opus allows a far superior voice input than possible on the


phone and existing audio codecs (G722, G729, Speex) with
an improved bandwidth consumption and shorter delay.

A bandwidth usage adapted to satellite


VP9 and Opus codecs are specially adapted to low bandwidths (500 kbps) and latency of around 600 ms as
often found on a satellite. In addition to supporting medical instruments’ quality, its exceptional performance
allows a drastic reduction in the cost of satellite communications.

Finally, ParsysRTC offers many options configurable by the customer in order to save on bandwidth:
• audio-only transmission;
• transmission at a different rate of each side: only the image of the patient is essential, the quality of the
image transmitted by the physician may be lower or eliminated if necessary;
• definition of a maximum usable speed either by reducing the resolution or the refresh rate of the image
(configurable on the server, rather than on the station for easier management).

Parsys MedCapture

Parsys MedCapture is a client interface running as a background


task on equipped workstations.

In a transparent and intuitive matter, it allows:


• Local data acquisition
• Dialogue with the different physical drivers on the devices
• Configuration of the devices if necessary
• Orchestration of the application builders when inseparable from the sensors
• Full access control of the remote station
• Transmission to Parsys Cloud for integration with electronic health records

Consequently, the software installation is a necessary prerequisite for all workstations equipped with
sensors.

Asynchronous Mode

Parsys MedCapture can be used in asynchronous mode. Once


the software launched, a medical dedicated Man-Machine
Interface (UI) shows up. The UI can be configured during the
installation specifically for managing of devices connected to
the workstation.

According to the requested sensor, Parsys normalizes the data in


order to insure an adapted visualization experience. For
example, when using a medical camera such as
ophthalmoscope, otoscope, dermatoscope, general exam, a
picture or a video is directly accessible in the expert doctor’s
web browser. Similarly, when using an oximeter, an oximetry
value is delivered in text and directly fills an EHR field. It works the
same way with other medical sensors.

Synchronous Mode

The expert doctor can remotely activate an UI in synchronous


mode. In such mode, the solution is able to support real-time
flows like streaming flow. Within a ParsysRTC video consultation,
the user can also activate the medical UI in streaming mode. In
order to respect the patient’s private life, the receiver must
accept the video request to share his screen and to allow a
remote access of his workstation. Once the authorization given,
the expert doctor can take full control (remote start and
stoppage of the instrument, real-time patient’s condition, use of
calipers in capture of an ECG, change of visualization or traces
resolutions, etc.).

Furthermore, at the end of a consultation, a Web service call


allows the sending of secured data, at appropriate formats,
which will be included into the EHR to insure traceability of the
medical act.
