Part 3: Contingency Plans. Standard PPE, S: First aid kit must be present at the area and the Hse personnel are the first response in case of
emergency. Man lift must be present all the time. Safety Observer is responsible to check the time.

Part 4: Review. After the job has been completed, JSA leader should note: any hazards, which were identified in the original JSA? Any practices
that need to be captured in the OMS? List action items, changes, and transfer any changes to the Change Management system.

Improper PPE’s Ensure that PPE’s shall be worn at all times like
hand gloves, safety helmet with chain strip, safety
goggles and coveralls
Poor Ventilation Ensure that the fans are must be in place for proper
ventilation of air and also Air compressor must be
use for fresh air
Entering without body harness Ensure that the entrant must be used the full body
harness and hook off body harness with the
lifeline.

Left the position of attendant Ensure the attendant or watch man must be present

Deficiency / Excess of Oxygen Confined space coordinator must be monitor every
30 minutes with gas detector
Confined space coordinator must ensure that the
oxygen limit is in between (19.5 to 21.5 %)

less.

MSDS shall be available and all the instructions
shall be followed
Respirators shall be worn while applying Foster
at the back side (Confined area) of the stripper.
Leather/Plastic gloves shall be worn while
applying Foster.

6. Demobilzation Poor House Keeping After completion of job Supervisor is responsible


for house keeping.
