DOLE OSH STANDARDS RULE 1020 - REGISTRATION
1021 General Provisions Every employer as defined in Rule 1002 shall register his business with the Regional Labor Office or authorized representative
having jurisdiction thereof to form part of a databank of all covered establishments

1022 Registrable Unit The establishment regardless of size of economic activity, whether small, medium, or large scale in one single location,
shall be one registrable unit.

(1) Existing establishment shall be registered within 60 days after the effectivity of this Standard
1023 Period of Registration
(2) New establishments shall register within 30 days before operation

(1) Registration shall be made in form DOLE-BWC-IP-3 in three copies and to be submitted to the Regional Labor Office
or authorized representatives.

(2) Registration shall be free of charge and valid for the lifetime of the establishment except when any of the following
1024 Registration conditions exists, in which case, re-registration as if it were a new establishment is required:
a. change in business name,
b. change in location,
c. change in ownership, or
d. re-opening after previous closing.

Appropriate training and certification by the Technical Education and Skills Development Authority (TESDA),

Equipment before use of equipment, if applicable. Trainings provided by international organizations or suppliers if not locally
available, shall be recognized by the DOLE
To ensure that the safety and health program is observed and enforced, a safety and health committee shall be
organized in covered workplaces.
For medium to high risk with 10 to 50 workers and low to high risk with 51 workers and above- The OSH committee of the
covered workplace shall be composed of the following:

Members - Certified first-aider, OH nurse, OH physician as applicable

For 2 or more establishments housed under one building. The joint Coordination Committee shall be composed of the following:

Members - At least 2 safety officers from any of the establishments
- At least 2 workers's representatives, one from the Union if organized

Secretary - Safety Officer of the building appointed by the Chairman

Minimum classification and number of safety officers for all covered workplaces shall be as follows:

Section 14 Safety Officer (6) BOSH / COSH Training Certificate for Designated SOs
Covered workplaces shall have qualified occupational health personnel duly complemented with the required (7) First-aiders certification by Philippine Red Cross
medical supplies, equipment and facilities. (8) Nurses certification by OHNAP
(9) Physicians certification by PCOM / MOA with hospital

All employers, contractors or subcontractors, if any, shall submit to DOLE all safety and health reports, and notifications
