Respectfully submitted to ENGR. CEDRICK R. BUTAO, CHDO-OIC,


this City, the herein Deed of Absolute Sale of the awardee Mr. George O.
Benigay with a location of Block No. 6, Lot 22, Furia Subdivison, Barangay
Libertad, this City.

As provided under Section 8 (a) of the Revised Rules and Regulations in


the Implementation of CIGLA Program, it is a requirement by the City
Government that the awardee shall “construct a residential unit or introduce
improvements on the awarded lot two (2) years after awarding, reckoned from
the acceptance of the Certificate of Award. “

After thorough review, the requirements for the execution of a Deed of


Absolute Sale, as provided by the CHDO, the awardee only complied the
following requirements:

1. Letter of Request for the Deed of Absolute Sale;


2. Lot Award;
3. Valid ID of the Awardee (Xerox Copy);
4. Certification of Full Payment as issued by the City Treasurer;
5. Tax Declaration;
6. Approved Subdivision Plan from the DENR (Blueprint);
7. Title;
8. Inspection Report conducted by the CHDO.

The awardee lacked Certification from the CHDO that would attest
whether the awardee actually build and occupies the lot or even introduce
improvements thereto. Thus, it is requested for the CHDO to issue a
certification as to the status of the awarded lot with regard the improvement
and occupancy thereof.

Furthermore, it is recommended that the Lot Award attached be a


Certified True Copy from your office. Anent to this, the Deed of Absolute Sale
appears to be proper as to form and substance, and in accordance to laws, good
morals, public policies and public order.
