INSTRUCTIONS:

1. Medical fitness certificate of each student admitted will be obtained from the University
Medical Officer.
2. Any incorrect statement made in this form may lead to disciplinary action or cancellation of
admission at any time.
3. (i) Attach attested photocopies of:
(a) Academic Certificates (b) Hafiz-e-Quran Certificates (c) National I.D. Card
(d) Domicile (e) Character Certificate from the Institution last attended
(f) First page of publications (g) Detail Marks Certificates of last examination passed
ii) Two Attested photocopies of all above mentioned documents (for PhD four copies of each
document)
iii) Four Passport Size Photograph (Attested)
4. At the time of admission following original certificates must be provided:
(a) Academic Certificates (b) Character Certificate the Institution last attended
(c) Hafiz-e-Quran Certificates (d) Detail Marks Certificates of last examination passed
(e) National I.D. Card (f) Domicile
5. If a candidate is offered admission and has paid the fee/dues in the University, then the
Fee/Dues once paid will be refunded/ transferred as per university rules.

UNDERTAKING:

1. I declare that I am not a member of any political party and that I shall not indulge in any
political activity as long as I remain a student of the University. I further undertake that I will
not challenge the finding /decision of Head of the Institution regarding my
Rustication/Expulsion from the University or cancellation of my admission at any stage
whatsoever in any Court, Tribunal, Authority or Forum other than the Supreme Court of
Pakistan.
2. I further undertake that I shall not claim hostel accommodation as a matter of right.
3. I hereby certify that I have myself filled in this form and the statements made herein are
