The client had pain at the surgical incision site and felt dizzy when walking or sitting. The nurse assessed the client for risk factors for infection and identified proper hand hygiene, isolation if needed, and a clean environment as interventions. The goals were for the client to understand infection risk factors and prevention, properly care for the wound, and heal without signs of infection. The nurse evaluated the effectiveness of the interventions.
