INCIDENT REPORT
 Position Indicator malfunction (June 23, 2018)
Date
 Bypassed Feeder Relay Protection (as stated)
Place Bauang Substation
Problem/Concern As stated.
 Upon giving the clearance to de-energize the PGL Feeder Last
June 23, 2018 (0702H) Scheduled Annual Preventive
Maintenance. We use the position indicator to control / isolate the
circuit breaker from the feeder but it failed to operate, so we
decided to isolate the line from the push button on the breaker
(medium voltage switch gear) to isolate the line.

 (A.T.O SF6) Last July 24, 2018 at 0216H & 0412H (1st & 2nd
encounters by passed PGL Feeder Relay Protection). The Operator
on duty conducted per equipment inspection for possible location of
fault inside the substation, but no identified possible equipment
failure. The troubleshooting also conducted site inspection on
distribution lines but reported “undertermined cause of trouble”.
 Alarm annunciator indicated "No. 14 - SPARE" instead of
“No. 13 – Lock Out Relay 1 TRIP” for SF6 outage.
 But PGL protection relay indicates phase to ground fault.
Alarm 1. (02:16h) ground time overcurrent 51G - G - 12.37 pu.
Alarm 2. (04:13h) phase time overcurrent 51P - B – 23.10 pu

Taken:
 (A.T.O SF6) Last August 28, 2018 at 0541H (3rd encounters by
passed PGL Feeder Relay Protection) – reported
“ELECTROCUTED BIRD NEAR VILLA STRELLA”
 Alarm annunciator indicated "No. 14 - SPARE" instead of
“No. 13 – Lock Out Relay 1 TRIP” for SF6 outage.
 But PGL protection relay indicates phase to ground fault.
Alarm 1. (05:41h) Ground Time Overcurrent 51G - G - 18.26 pu.

 (A.T.O SF6) Last September 14, 2018 at 2342H (4th encounters


by passed PGL Feeder Relay Protection) – The Operator on duty
conduct per equipment inspection for possible location of fault
inside the substation but no identified possible equipment failure.
reported “DUE TO INCLEMENT WEATHER CONDITION
(TYPHOON UMPONG)
 Alarm annunciator indicated "No. 14 - SPARE" instead of
“No. 13 – Lock Out Relay 1 TRIP” for SF6 outage.
 But PGL protection relay indicates phase to ground fault.
Alarm 1. (23:43:34h) phase time overcurrent 51P - A - 19.99 pu.
Alarm 2. (23:43:34h) ground time overcurrent 51G - G - 13.64 pu.

 (A.T.O SF6) Last September 17, 2018 at 0839H (5th encounters


by passed PGL Feeder Relay Protection). Reported to emo that the
SF6 trip, then emo gave clearance to energize the line at 0840H.
The operator on duty reported unbalanced readings and alarms
pick up by PGL Feeder.
 Alarm annunciator indicated "No. 14 - SPARE" instead of
“No. 13 – Lock Out Relay 1 TRIP” for SF6 outage.
 But PGL protection relay indicates phase to ground fault.
Alarm 1. (08:40:58h) ground time overcurrent 51G - G - 10.71 pu.
Alarm 2. (08:41:00h) ground time overcurrent 51G - G - 16.25 pu.
Alarm 3. (08:42:06h) ground time overcurrent 51G - G - 03.11 pu.
Alarm 4. (08:43:48h) ground time overcurrent 51G - G - 15.50 pu.
 At 0853H EMO request to De-energize the PGL Feeder due to
reported cut primary line along pagdalagan bridge. Upon de-
energization, the position indicator failed to operate, so they use the
push button to de-energize the feeder, (same situation on
energization of PGL feeder energization at 1041H).

Cause of trouble found: “DUE TO CUT 1 SPAN (PHASE B)


PRIMARY LINE AT PN 50 TO PN 51 PAGDALAGAN BRIDGE &
INDIAN TREE (Polyalthia longifolia) INCONTACT WITH PRIMARY
LINE (PHASE B) BETWEEN PN 246 TO PN 247.”

 to troubleshoot the malfunction PGL position indicator or a


replacement of position indicator.
 to adjust the feeder relay data on PHASE & GROUND trip pickup
Findings & values.
Recommendation  to check the alarm monitor indicator connections or move the
description “LOR 1TRIP to no. 14” and use the no. 13 as “VCB PGL
TRIP” and to tap the PGL,CBL & PYL on its buzzer.
 to conduct field or site inspection before energization..
