Learning Feedback Diary1

The student completed a two day exposure with a family in the community. During this time, the student collected health information from the family including their family history, health history, and conducted an OB assessment of the pregnant mother. After analyzing the collected data, the student identified relevant health problems and areas where additional information was needed. This experience highlighted the importance of understanding a family's full health history to identify current and future health issues. The student reflected on their experience and how community health workers assist pregnant women in the area. The experience was guided by Faye Abdellah's 21 nursing problems which focuses on a patient-centered approach and appreciating social factors that can influence health.

OBJECTIVES: From the plan of activities

5. Specifies client’s status/conditions/problems to be addressed identifying reasons of
existence.

CONTENT:
Described completely the experience from day 1 to the last day of exposure
1. What I have learned? (There is a deep and broad explanation fully supported and verified
of learnings)
On the second and third day of my exposure to the family, we gathered information using
the first and secondary review of data. I was able to collect starting from the family history,
Health history, and other information needed. We also assess the family especially the
mother who is pregnant and use OB assessment tool. After I collected all the data, I sorted
all the data and clustered all the cues that I identified. I determined that the data can be
relevant and irrelevant as well. Then, we started to compute the identified problems and
realized that there are still lacking information that we needed.

2. How will this experience change my practice?


In my 2 days encounter with the family, I realized how important it is to know your family
health
history and other information for you able to identify the current problem and what will be
the
future problem of the whole family.

3. Reflection questions surrounding the activities rendered ( point of clarification then this can
be a point of discussion to students)
There are community health workers from the barangay halls that make sure that all
pregnant within their area is assisted by going to each household and encouraging them to
visit nearest health center for follow up check-up. This helps the health facility to record all
pregnant women that are residing to the area and monitored their health.

4. What nursing theories help and guide me in rendering my nursing care to the patient and
family.
Faye Abdellah’s 21 nursing problems changed the focus of nursing from disease-centered to
patient-centered it relate to change and anticipated changes that affect nursing. The need to
appreciate the interconnectedness of social enterprises and social problems; the problems
such as poverty, pollution, education, and so forth that effect on health of the family.
