SCHOOL HISTORY
San Agustin Elementary School, its mother barangay is the O’Donnell, Capas, Tarlac. The
school ID of the said school is 160017 it is located approximately 26 km from the town proper.
The school has a total land area of less than 5000 sq. meters and school population of 85
pupils. It has 4 classrooms with 4 comfort rooms for children. The school has covered court used
in time of programs and graduations. The school is powered by electricity from Tarelco and its
water supply run by NAWASA. There are also provision of the improvise hand washing and tooth
brushing facilities for pupils health care program. The school is bless to be one of the
beneficiaries of the school-based feeding program, this program of DepEd is a big help for our
pupils, it eliminates numbers of pupils under wasted and severely wasted level of nutritional
status.
There are only 4 teachers for 7 classes from kinder to Grade VI. Each teacher is handling
multi grade classes except the kinder teacher. There are three teachers who belong in Teacher I
position. One teacher is presently enrolled in doctoral degree. Only one teacher belongs to
As of their ages, the youngest is 25 and the oldest is 52. This means that the teacher were
very much capable in handling multi-grade classes because they are still active and energetic.
Most teachers were all advance in using ICT. When it comes to submission of reports, all are

To the situation of the learner: Access-Attendance and Retention. All school aged children
are attending the school. Quality Grade IV, Grade V and Grade VI were all promoted. They have
100% promotion rates for the past 3 years. In reading 5 were under frustration level; 12 were
under instructional level and 27 were under independent level in English while in Filipino we have
0 frustration readers; 22 were instructional readers and 39 independent readers base on the Post
