INDIVIDUAL MEMBERSHIP APPLICATION

An Individual Member pertains any individual of good moral standing in the community, engaged professionally in quality
management work or business.
1. Individual membership privileges are non-transferable
2. Individual members will receive emails or invites for any PSQ related information or activities, promotional and informational materials
3. Annual membership fee is Php 1,500

- New memberships are effective upon the approval of the PSQ Membership Committee and PSQ President
- New members receive one year of membership from the date they paid their annual membership fee.
- New members will be invited to an Induction Ceremony during during the General Membership Meeting
- Members are billed prior to the anniversary date of their membership for succeeding year's dues.

CORPORATE MEMBERSHIP APPLICATION

A Corporate Member is any company, corporation, partnership, association, proprietorship, etc. of good standing and operating in the
Philippines.
1. A corporate member is entitled to appoint a primary representative and an alternative representative
2. Both representative will receive emails or invites for any PSQ related information or activities, promotional and informational materials
3. Annual membership fee is Php 4,200
4. The primary representative shall be responsible in sharing the information on the Corporate Membership with the other employees of the member
organization
