Dear Sir:

This letter is in reference to the letter sent by Red Cross regarding the proposal to conduct
Blood Letting activity. The said activity was being undertaken in cooperation with our university thru
its NSTP CWTS and ROTC. With this I would like to request permission to allow us conduct a Blood
Letting activity on April 29, 2019 from 8:00 am to 4:00 pm at the CHMT function Hall. The blood
donor shall come from the NSTP CWTS and ROTC students. Interested faculty members, employees,
and other students may participate as blood donors.

In addition, I would like to request an amount of three thousand pesos (PhP 3,000.00) from
the NSTP CWTS fund for the refreshments of Red Cross volunteers.
