 INSTRUCTIONS

1. The candidate must have been enrolled as a regular student for the session in the affiliated
college and registered with Quaid-i-Azam University, Islamabad (QAU).
2. Fill the examination form yourself in your own handwriting using blue ballpoint. Cutting,
overwriting and use of correction fluid are not allowed; in complete/incorrect forms shall not
be accepted. The form must be attested and forwarded through the College Principal.
3. Following documents must be attached with the form:

i. Attested photocopy of SSC or equivalent with the IBCC equivalence certificate.

ii. Attested photocopy of HSSC or equivalent with the IBCC equivalence certificate.
iii. Attested photocopy of result card issued by QAU (if applicable).
iv. Attested photocopy of registration card.
v. Attested photocopy of CNIC/Form B.
vi. Original bank receipt of fee deposited in campus branches.

4. The candidate must sign the form himself/herself (to be assured by the college) in case of
variation in signature on verification the candidate may face impersonation charges.
5. Examination form and fee for the Part-I and the Part-II examinations must be submitted
separately; no candidate shall be allowed to appear in the both parts on single form/single roll
number
6. Examination form alongwith original bank challan for normal fee reach the office of the
Controller of Examinations on or before the due date. Late fee shall be charged according to
the announced schedule and the following fee structure (may be changed by the University
authorities from time to time). Fee once deposited is neither refundable nor adjustable.
