About Google Forms
• Google Forms, another product from Google, is an app to create web based forms and collect
answers online.
• As a platform, Google Forms can be used for practically any data gathering purpose. It is
popular to use Google Forms to develop online questionnaire and to conduct surveys. Google
Forms is useful for students doing school projects as well as professionals for company
projects.
• Google Forms has quiz feature that enable question scoring. This feature can transform
questionnaire into learning/educational tool that can be very useful for teachers or trainers
for example to administer quiz/test to their students or course participants through their
devices (smartphone, tablet or laptop).
• Google Forms is Free.
• This tutorial is for Google Forms to create online survey. Google Forms for quiz is in part 2 of
this tutorial.

Interest to Buy a New Smartphone
• This tutorial contains an example of Google Forms questionnaire, where the objective is to
understand consumer interest to buy a new smartphone
• In the questionnaire, we want to show picture of the phone and its specification
• As background to the finding, we want to understand brand awareness, brand image and
ownership.

• Final questionnaire will be shown first along with description for each question, then
followed by how to make the questionnaire.

In “Linear Scale” respondent is to choose a number. Usually it is a question on evaluation about something represented by a number. Select
range of values (lowest 1 and highest is 10) with corresponding labels for lowest and highest number.

When you start developing questionnaire, a section is
automatically created.
Section is important in two ways :
1. The display in device screen is by page, one page
for a section. A section can contain all questions in
the questionnaire or just one question or one
title. Some project owners prefer to show
questionnaire in one page that to go to next
question you do that by scrolling, others prefer
one question at a time, have it answered and
