Subject: An Incident for Construction of Pipe Pile No. PP96

We refer to a routine inspection at 13:30 today and observed that the pipe pile no. PP96
was drilled down the hole near Gage Street. When the steel casing reached the toe level
at -2.50mPD, the drilling operation could not continue to further go-ahead to the tentative
casing toe level at -13.00mPD. The steel casing was terminated at 10.50m above the
tentative casing toe level. Meanwhile, your foreman explained that the casing toe and drill
bit were detached from the steel casing.

We record the existing condition of pipe pile no. PP96 was listed below:
Casing length: 25.64m (original 30m long and cut top 4.36m)
Casing top level: +23.14mPD
Casing toe level: -2.50mPD
Casing tentative level: -13.00mPD

You are required to investigate the causes of an incident and submit a remedial proposal
for our review.

You are reminded of your duty as registered contractor under Building (Administration)
Regulations 41 and are required to strengthen the site supervision to prevent further
occurrence of the similar incident.
