SUBJECT: APPLICATION FOR MONETIZATION OF LEAVE CREDITS/PROVIDENT

1. To facilitate the process of monetization of leave credits, please be informed that


all requests shall be filed every 1st week of every quarter.

2. School heads and personnel who wish to avail any type of leave monetization
should fill up the application for leave form (CSC Form 6) and shall submit it on
or before ______________________ to HRMO for certification of leave credits.

3. The following requirements are requested:

a. CSC Form 6 (3 copies)

b. Medical Certificate (CSC Form 41)/Clinical abstract/medical


procedures to be undertaken (for medical), Barangay Certification
(calamity), Letter Request who will avail 50% of the leave credits
monetization (3 copies).

4. Immediate and wide dissemination of this Memorandum is earnestly desired
