Exit Interview Form

The exit interview form collects information from departing employees about their reasons for leaving, satisfaction and frustrations with their job, recommendations for improving employee satisfaction, and whether they would consider returning to or recommending the company. Employees provide open-ended responses to questions about their primary reasons for leaving, most and least satisfying aspects of the job, and suggestions for improvements. The interviewer also comments and indicates whether the employee is eligible for rehire.
