open in MS Excel 2000 or higher version


PARTICULARS SHOULD BE REMARKS

period covered of remittance:


e.g. April 2015 201504 format is yearmonth(YYYYMM)

lastname/name/midname:
NIÑO NINO do not use character( Ñ ñ )
MA. NIÑA MA NINA do not use period and comma

Pag_IBIG ID/RTN:
1211-4550-2344 121145502344 do not use spaces/hypens etc
9150 1234 9876 915012349876 this should be 12 digits , no more, no less

Additional Notes:
1. Pls use and submit different excel file for each type of remittance (CONT/MPLoan/Calamity)
- do not mix remittances in one excel file/sheet

2. Pls use dropdown menus at the rightmost of column for Membership Program, Loan type
when filling up the said cell with appropriate type.

3. Do not add, delete, alter or modify labels/ columns. preserve the format of the file.
