Dear Ma’am/Sir,

I am a registered midwife and I am writing to apply for the Midwife I position under the
Municipal Health Office in the municipality of Governor Generoso, Davao Oriental.

I worked for twelve (12) years as a midwife hired by the Local Government Unit, assigned in
Brgy. Tandang Sora, Gov. Generoso, Davao Oriental. Then currently working under the
Department of Health as a midwife of Rural Health Midwife Placement Program (RHMPP)
from the year 2012. I am currently assigned at the birthing facility of Governor Generoso,
Davao Oriental and I am greatly interested to continue rendering services under the
Municipal Health Office.

I believe my experience as a midwife is a valuable asset for the position. I am trained with
Basic Emergency, Obstertric and Newborn Care ( BEMONC), Family Planning Competency-
Based Training-I (FBCTBT-I) and Basic Life Support (BLS) which are essenctial for the renewal
of Maternal Care Package (Paanakan). I work very well and go out of my way to give quality
care to my clients.
