TEACHERS’ EVALUATION ON THEIR DUTIES AND RESPONSIBILITIES


(STUDENT - Evaluation Form)

Please make serious evaluation. Read each statement of characteristics under each factor
thoroughly and comprehensively so as to arrive at a “sound and fair judgment” to your teacher.
Choose the factor of his/her performance. Do not omit any factor, refer to the given weight or
adjective rating. The choices of your judgment are of 5 scales and select only one of those
alternatives namely: Excellent (5), Very Good (4), Good (3), Fair (2) and Needs
Improvement (1), taking into account the Teacher as classroom manager, as implementor and
values or attitudes, please rate on a scale 1-5, with 5 as the highest.
Please check the column you so desire.
