Reflection Paper For Pharmacology

The episode follows the case of Gabriel, a young Congolese boy with severe congenital heart defects. Dr. Melendez's team is unsure if they can help him. Dr. Murphy realizes a possible procedure to move the mitral valve into position. After practicing, Dr. Melendez performs the high-risk surgery, which is a success. The reflection notes that doctors are expected to save lives, even with low odds of success, if that is the patient's choice. Saving lives extends beyond just treating disease.

Curing the Disease of the Society

Since the reign of President Rodrigo Roa Duterte the issue with regards to “war on drugs” was
deeply associated to him. According to Human Rights watch at least 2,555 of the killings have been
attributed to the Philippine National Police. Furthermore, Pres. Duterte and other senior officials have
instigated and incited the killings in a campaign that could amount to crimes against humanity. Up until
now, news about buy bust and entrapment operations are flooding the media where they showed factories
of high end drug dens which can or is able to produce tons of kilos of shabu are being destroyed and
burned, some operators of this Shabu laboratory are aliens.

During the 3rd State of the Nation Address (SONA) of President Rodrigo Roa Duterte, allegedly
a number of deaths in the President’s intensified anti-drug campaign rose to more than 4,500.According
to Philippine National Police (PNP) documents there are 4,540 drug suspects have been shot dead in anti-
drug operations, all presumed by cops to have fought back ("nanlaban"). The data cover the period of
July 1, 2016 to June 30, 2018. There were also 149,265 people arrested and 1,274,148 who surrendered,
according to the PNP, this statistics was release by PDEA . Because of this War on drugs the executive
branch has been intimately focusing, several prisons are flooding with drug cases and prisons are
overcrowded.

However despite of this safeguards our country have been providing to eradicate Drugs which
destroyed many families and lives, a huge number of the Philippine population is not sure of the longevity
of the current safeguards provided. Currently a bill is being read in congress re-imposing Death Penalty,
which included Drug related case which such penalty could be applied. As a result several human rights
advocate and the church strongly conveyed their opposition in the proposition.

As a Medical Student and a Citizen of this country I have witness in my own bare eyes several
lives that where destroyed by this so called “Prohibited Drugs”. Many Crimes such as Rape, robbery and
killings were deeply associated with the use of prohibited drugs of the perpetrator and some even
associate poverty with drug addiction.

A movie entitled buy-busted which showed a situation where the several high ranking police
official, drug lords and even the whole barangay conspired to plot the death of several police men to
falsely present to the country the false effort to eradicate drugs, while such premise may be considered as
fiction to some, others may believe that such story is true. The notion that the PNP is part of the
instigation of crimes is not impossible. Because of such idea congress should bestowed its sharpest fangs
upon those who violates crimes that destroyed not only the life of an individual but have greatly affected
the lives of many. As future medical practitioner I am under the impression that saving lives is not only
limited to aiding and curing diseases it extends to aiding social diseases that kills and endangers our
society and one way of aiding this social diseases is to support means that can eliminates the cause of
such.

SEASON 1; EPISODE 9 : Intangibles

The episode started with the case of Gabriel a young boy from the Congo who has severe congenital
heart anomalies. The case was referred to Dr. Melendez and his team by Dr. Mehta. When Dr. Melendez
checked the heartbeat of Gabriel he knew that the child’s case was hopeless, however despite of the
obvious observation they conducted tests to prove what they have expected. He then approached the
mother of Gabriel informing her that they cannot do anything about the child’s case and told her that they
will provide medicines for the child and after which they could go home. However, the child had an attack
which the doctors diagnosed as endocarditis but his echo revealed vegetation in the valves or
endocardium and he has not develop a fever Dr. Mehta then ordered that to be sure to get blood culture.

While the child was playing his potato head toy Dr. Murphy suddenly exclaimed “it’s in the wrong
place” and grab the toy and placed the face of potato head in its proper order. However, he looked at the
toy again and grabs the toy and ran. He suddenly swift his way to the laboratory telling Dr. Melendez to
move the mitral valve and pushed the toy toward Dr. Melendez’s face, he then replied “ you can’t move
the mitral valve” but Dr. Murphy said “ I believe that full insertion of the mitral valve can be..” Dr. Melendez
stopped Dr. Murphy and ordered him to follow him. They then talk about the septal myectomy can put the
mitral valve to a normal position. Consulting the head the hospital, the head asked Dr. Melendez if he is
able to conduct this procedure, he replied “it’s a terrible idea”. During the course of their conversation, Dr.
Melendez’s deeper deep indicating that the child’s condition is getting worst. The doctors then said that
the meds where not working and Dr. Murphy said that maybe the arrythmia is a manifestation of Gabriel’s
LV dysfunction. However Dr. Melendez fathom that the mother is not giving Gabriel his medicines. In
confrontation between the mother and Dr. Melendez, the latter realize that he may save the life of the
child.

Dr. Melendez then called Dr. Murphy and asked him to try to replicate the procedure they will do
to Gabriel, after many trials they finally perfected the procedure and decided to conduct the surgery which
was successful.

Reflection:
In the field of medicine Doctors are expected to save lives. Doctors invested many years of their
lives to save other people’s lives and the thought that a patient may die in their hands in the middle of the
procedure taking the days the patient is suppose to live is a fear every Doctor is terrified off.

In this episode Doctor Melendez was fearful that he might kill the child however the mother put
faith on him. Despite the low probability of the success rate of the operation, Dr. Melendez conducted the
operation and took his chance.

The choice is always in the hands of our patients and when the patient still chooses to continue
despite the very low probability of the success rate the doctor’s duty is to give his full potential to help
them.
