This is to certify that I am allowing my son, Juland A. Padilla to go on a practicum (on-


the-job training) for a minimum of 300 hours starting on April 10, 2017 until June 02, 2017 at
Engineering and Development Corporation of the Philippines in partial fulfilment of the
requirement for the degree in Bachelor of Science in Civil Engineering.

It is understood that abides by the rules and regulation that may be imposed by the
supervisor/Staff-in-charge for his welfare and safety.

I fully agree to waiver any responsibility on the part of the College of Engineering-
Polytechnic University of the Philippines, Engineering Development Corporation of the
Philippines. and/or the representative/s, in case of any untoward incident that may happen to my
son during the duration of the practicum.
