the Platinum
Membership
Application Form
Log on to:

and click “Upgrade to Platinum”

Supplier Information

Must be the same with the Organization Name registered in the


PhilGEPS database.

The username that the supplier uses to log in to the system.

Name of the supplier who will process the Platinum Membership


application of the company.

Must be valid, active, and accessible. All notifications regarding the


application will be sent to this email address.

Indicate whether new platinum membership application, update or


renewal

Form of Organization of the user upgrading to Platinum


 Type of Application
- New Application – Upgrade of Membership from Red to Platinum
- Renewal – Renewal of Platinum Membership
- Update of Document – Updating of documents submitted for their Platinum
Membership.

 Form of Organization
- - Cooperative
- Corporation/ Partnership
- Individual Local Consultant
- Single Proprietorship
- Foreign Company/ Corporation
- Individual Foreign Consultant
New Application/ Renewal

 Select the Form of Organization for of their company in the


dropdown list.
 Fill in the required information based on the selected form of
organization.
Update Documents

 Tick the checkbox to select the document you want to update.


 Fill in the required information in the text fields.
DTI Certificate Details

Certificate No. in the DTI Registration Certificate

Name of DTI Secretary appearing on the certificate

Registration/ Issuance of the DTI Registration Certificate

Expiration Date of the DTI Registration Certificate


SEC Certificate Details

Registration/ Issuance date of the SEC Certificate

Certificate No. in the SEC Certificate


CDA Certificate Details

Registration/ Issuance date of the CDA Certificate

Certificate No. in the CDA Certificate


Business Registration Certificate
(Foreign Company/ Corporation )

Registration/ Issuance date of the Business Registration Certificate

Business Certificate/ Registration Number in the Business Registration

Mayors Permit Details

The place where the Mayors Permit was issued.

The permit number indicated in the Mayors Permit.

The date when the Mayors Permit was issued.

The person who signed the Mayors Permit. It could be the Mayor or
Authorized Signatory in the Mayors Permit.

The validity or expiration date of the Mayors Permit.


Tax Clearance
(17.14B - For Bidding Purposes)

Date when the Tax Clearance was issued.

Person who signed the Tax Clearance or Authorized Signatory.

TCC No. specified in the Tax Clearance

Validity or expiration date of the Tax Clearance


Audited Financial Statement (AFS)

Date in the stamp received by the BIR

Current Asset specified in the Financial Statement of the AFS

Total Asset specified in the Financial Statement of the AFS

Current Liabilities specified in the Financial Statement of the AFS

Total Liabilities specified in the Financial Statement of the AFS

Auditor in the Auditors Independent Report in the AFS

The BIR RDO Code in the AFS


PCAB License (For Contractors ONLY)

Expiration Date of the PCAB License

Name of Board Member appearing on the certificate

Issuance Date of the PCAB License

License No indicated in the PCAB License

Date when the license was first issued

Principal Classification indicated in the PCAB License

Category indicated in the PCAB License


Sworn Statement

Date when the Sworn Statement was signed.


Individual Local/Foreign Consultant

Date Last Updated: Date when the Curriculum Vitae was updated.

Date Signed: Date when the Sworn Statement was signed.


Uploading of Documents

 After filling out all the required information upload all the Eligibility
Documents by merging all the file into a single ZIP or RAR file. The ZIP or RAR
file must not exceed 10mb.
Submitting the application

 Review all entered information and click submit button.


Reminders

 Documents shall be reviewed and approved by the PhilGEPS


Administrators. Please allow 5 working days for processing of submitted
documents.
 Payment of P5,000 shall be made only after approval of submitted
documents.
