Note: 1.Please submit the application form duly completed to Director (Numbering & Type Approval) on above address together
with the following:
a) Application fee
b) All items specified in the checklist given in Section C below.
2. Incomplete applications will not be accepted.
3. Payment should be made by a Pay order / Demand Draft in favour of PTA to PTA’s collection account No. NIDA 11-1 at
NBP, Jinnah Avenue Brach, Blue Area, Islamabad, under written intimation to DG (Service Division), PTA. The application
fee is not refundable.

Section C: Submission Requirements


Checklist for submission:
[ 1 ] Undertaking from the applicant that Pakistan Telecom Act 1996, Policies ,Rules and Regulations and directives will be
followed.
[2] Board Resolution authorizing CEO, Director or any employee to submit application / related documents to PTA and deal with
PTA to get the Type Approval.
[ 3] Fees (Application Processing Fee and Type Approval Fee as per PTA Type Approval Regulations) will be paid on time.
[ 4 ] Technical Data and Specifications.
[5 ] Test Reports and Declaration of Conformity or Certificate of Compliance to be issued by Accredited Laboratories regarding
following:
a. Electromagnetic Compliance (EMC) test report b. Electromagnetic Interference (EMI) test report c. Safety
test report d. Specific Absorption Rate (SAR) test report e. Radio Frequency (RF) test report.
[ 6 ] Certificate of IMEI from GSMA Association and MEID from CDMA.
[7] User Manual.
[ 8 ] Authorization Letter from Manufacture (where needed).
[9] List of Customer Service Centres established / to be established in Pakistan.
[10 ] Quantity and IMEI Serial Numbers (for each consignment).
