CHILD CARE PROGRAM:

1. Infant given BCG vaccine TP x .03 95%


2. Infant given DPT1, DPT2,DPT3 vaccine TP x .03 95%
3. Infant given OPV1,OPV2,OPV3 vaccine TP x .03 95%
4. Infant given Hepatitis B1 within 24 hours after birth TP x .03 95%
5. Infant given Hepatitis B1 more than 24 hours after birth TP x .03 70%
6. Infant given Hepatitis B2/Hepatitis B3 vaccine TP x .03 95%
7. Fully immunized children TP x .03 95%
8. Measles coverage of children under one year of age. TP x .03 95%
9. Infants 0-6 mos. old exclusively breastfed TP x.015 95%
MATERNAL CARE PROGRAM:

1. Pregnant women with 4 or more prenatal visits TP x .035 80%


2. Pregnant women given TT2 plus TP x .035 80%
3. Postpartum women with at least 2 postpartum visits TP x .03 80%
4. Postpartum women initiated breastfeeding within 1 hour after TP x .03 80%

NUTRITION PROGRAM:
