new Star readings on the first Star Chart in a different colour in order to get a snapshot of progress.
If your agency has its own paperwork for recording notes and action points then just complete the
Star Chart.

have come up in the discussion that you want to record. It can be completed by you or your worker
if you prefer.
If you and your worker have different views on the score for any area you can also use this space to
