AFFIDAVIT OF CONSENT

I, LETTY ANG LERIO, of legal age, Filipino citizen and a resident of L. Barrios St.,
Kalibo, Aklan, after having been duly sworn to in accordance with law, hereby depose and say:

That I am the heir of my deceased mother LUZ ARMIZA-ANG and therefore the new
owner of a two (2) storey commercial building located at Roxas Avenue, Kalibo, Aklan which
my said mother contracted for lease in favor of ELIZABETH A. LIMA, likewise of legal age,
Filipino citizen, and a resident of Laguinbanwa West, Numancia, Aklan, by virtue of Contract of
Lease entered in the Notarial Book of Immanuel L. Sodusta as Doc. No. 113; Page No. 24; Book
No. XIV; Series of 2012;

That it was stated in the said Contract of Lease that the Lessee can only assign or
sublease the said commercial space with my expressed consent and approval in writing;

That the lessee is desirous to sublease the said commercial space particularly the 1st room
at ground floor building to Gerome Trieste and that I hereby expressly give my consent to the
same and agrees that the monthly rental of the said room shall be Two Thousand Pesos
(2,000.00);

That I give my consent and authority to the said ELIZABETH A. LIMA to secure
necessary documents, permits, clearances, concerning the said commercial space including
mayors permit;

That I am executing this affidavit of consent to attest to the truth of the foregoing facts.
