Behaviour Management Sheet For Students

The document is a student reflection sheet that contains questions for a student to reflect on their behavior that resulted in being given the reflection sheet. It asks the student to describe what they were doing, how the teacher would describe their behavior, which needs they were trying to meet, and if their behavior was effective, respectful, and moving them in the right direction. It then prompts the student to list better choices they could have made and things they can commit to doing to improve their behavior. Finally, the student and teacher sign off on an agreed upon plan.
