(For the Purpose of Correction of Entry in the Birth Certificate)
I, JULIE DALANON DAYAP, of legal age, Filipino , single and a resident of
Poblacion, Taytay, Palawan, after being sworn to in accordance with law hereby depose
and state that:
1. That as of the present time, I am not employed in any capacity at any
corporation, company, and individual person;
2. That I have applied for the Correction of Entry in my Birth Certificate , specifically,
of Sex/ Gender in the Local Civil Registrar Office of Taytay, Palawan;
3. That I am executing this Affidavit of No Employment to attest to the truth of the
forgoing and to support my application for correction of entry in my birth
certificate in the LCRO of Taytay, Palawan.
