LETTER REQUEST
Sir:
We are requesting a three (3) days work permit for re-test of hydro testing of fire
protection system including the waterline connected to booster pump and diaphragm tank for
testing & commissioning, in which we schedule on February 6 to 8, 2014. The equipment we
used and should be entry/gate pass in the site (Plant) are 1unit Pressure Test Pump and 1 set
Fusion Machine. The following employee to be work:
