DOH-BHDT (Radiation Clearance for telecom/cell site projects)
QC Subdivision Admininistration Office (if townhouses or subdivisions)
Bldg. Permit, Certificate of Occupancy & Plans of existing buildings shown in the Vicinity
Map (if project is for renovation / for addition or for extension)
Contractor's / City Tax (pre-requisite for fast track applications)
