Thank you for using the COMELEC Online Application Form Submission (OAFS) System. Your REFERENCE NO. is
OAF740444000969481732R. Do not lose this number. You cannot reprint your accomplished application form without it.
Your appointment date is on 9 July 2015 (Thursday) 03:00 pm - 04:00 pm. Please bring the printed copy of your accomplished application
form to QUEZON CITY, 2ND DISTRICT, NCR - SECOND DISTRICT on that date.
Failure to appear on your scheduled appointment date shall automatically cancel your Online Application Form, which means you will need to fill-up
anew in the COMELEC website at https://irehistro.comelec.gov.ph/. if you wish to apply again.
However, you may still go through the usual manual process on any regular working day by (1) simply accomplishing the Online Application
Form.(without setting an appointment); (2) downloading a blank form and manually accomplishing it; or (3) ask for the usual forms to manually
accomplish at your local COMELEC office.

IMPORTANT REMINDERS TO APPLICANTS FOR REGISTRATION

PLEASE REMEMBER that accomplishing this Form online DOES NOT mean that you are already a registered voter. You still have to go
through the remaining two (2) steps:
a.

Go to your local COMELEC office to personally submit your Application Form. (This completes the process of submission of your
application)

b.

The Election Registration Board (ERB) has to approve your application on 19 October 2015 (Monday).

Printing Reminders:
a.

Be sure that you printed the accomplished form on a legal size (long) bond paper (8 x 14): If not, please reprint it.

b.

Make sure also that you have printed the VOTING RECORD page at the back of the COPY FOR THE ELECTION OFFICER as
instructed. If not, please reprint for compliance.

Do not sign or affix your thumbmark on the Application Form yet. (You have to do this in the presence of the Election Officer when you
go to the local COMELEC Office for filing purposes.)

4.

Your application is considered NOT FILED unless you submit it personally to your local COMELEC office and subject yourself to the data
capture of your biometrics information (photograph, fingerprint, and signature).

5.

Please bring ANY of the following VALID IDs : (Bring a photocopy of the required documents together with the original copy.)
a.

Employees identification card (ID), with the signature of the employer or authorized representative;

b.

Postal ID;

c.

PWD Discount ID;

d.

Students ID or library card, signed by the school authority;

e.

Integrated Bar of the Philippine (IBP) ID;

f.

License issued by the Professional Regulatory Commission (PRC);

g.

Certificate of Confirmation issued by the National Commission on Indigenous Peoples (NCIP) in case of members of ICCs or IPs;

h.

SSS/GSIS ID;

i.

Senior Citizens ID;

j.

Drivers license;

k.

NBI clearance;

l.

Passport;

m. Any other valid ID.
