Service Contract

This contract is between Crisanto F. Pulga, a carpenter and masonry worker, and the Hibabaogan Farmers Association (HFA) to provide labor for the construction of a processing shed. Pulga will provide tools, equipment, labor workers and helpers to undertake 12 scopes of work including site preparation, building installation, painting, roofing, foundation, and flooring. The HFA will oversee and monitor the work to ensure quality and completion. LABOR CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This contract executed by and between:


CRISANTO F. PULGA, (Master Carpenter and Masonry Worker) of legal
age,
Filipino,
with
residence
at
______________________________________________ hereinafter referred to as
the Contractor;

HIBABAOGAN FARMERS ASSOCIATION with office address at So.


Hibabaogan Brgy. Washington, Catarman N. Samar, herein represented by
the Association President ENGR. JAIME P. GILBUELA as the Community
Based Organization (CBO) of PCA KAANIB PROJECT, herein referred to as
HFA-CBO;
voluntarily binding themselves to the following:

1. That the contractor shall provide the necessary tools and equipment
and the needed labor workers and helpers for the construction of
KAANIB PROCESSING SHED at site;
2. That the contractor shall undertake labor for the following scope of
work:
a. Site preparation, clearing and levelling
b. Building layout and material hauling to site
c. Installation of 9 columns
d. Installation of wooden beams and roofing frames
e. Painting of wood parts with solignum and coaltar
f. Installation of G.I. sheets roofing and ridge rolls
g. Concrete pouring of zocalo foundation around building line
h. Floor backfilling and compacting to designed elevation
i. Floor concreting for the whole area
j. CHB installation over zocalo and to designed level
k. Installation of interlink wire as per plan
l. Fabrication of door panels and installation with door knobs and
hinges in place
3. The representative of HFA-CBO shall undertake the supervision and
monitoring to insure completion and quality of workmanship.
4. That PCA shall provide the funding and shall release the approved
amount of this labor contract upon submission of complete
documents, attested by PCA Authorized Representative.

5. That the approved contract amount for Pakyaw labor of the above
12 scope of works is FORTY EIGHT THOUSAND FIVE HUNDRED
PESOS (Php 48,500.00)
