This document is a bank account registration form for an investor to register multiple bank accounts, change an existing default bank account, or delete existing bank accounts associated with their investment folio. Part A allows the investor to add new bank accounts for receiving redemption or dividend proceeds. If you are changing an existing bank account with a new one as a default bank account for redemption/dividend proceeds in future, please mention the
new bank account in both Part A as well as in Part B. If the new bank account is not mentioned in Part B, redemption/dividend proceeds will be sent to
existing default bank account only. For each bank account mentioned in Part A, Investors should submit originals of any one of the documents
mentioned below. If copies are submitted, the same should be attested by the Bank or originals should be produced for verification.

Deletion of an existing default bank account is not permitted unless the investor mentions another registered bank account as a default
account in Section B of this Form.
