This part below is considered understood and is binding after you sign:
1. Participants will be authorized to bring the payment to EduPro, Inc. otherwise noted.
2. Reservation: One official Reservation Form for every course and schedule only. No reservations will be accepted without this signed form. Kindly
sign to complete this form and prevent processing delay.
3. Training Dates: Whole day training
4. Training Time: Time stated above is inclusive of 15 minutes AM and PM break each and 1 hour lunch break
5. Statement: Kindly request for the advance copy of the Statement of Accounts for check processing if necessary. Although please note courses may
not push through as scheduled unless confirmed.
6. Pre-Training: Please make sure the participants have read the course outlines prior to the training. Also require them to bring extra clothing since
our training rooms are sometimes very cold.
7. Confirmation: A Confirmation Notice will be faxed to inform you that seat/s has been reserved. Expect this at the earliest seven days before
training. Seats are not guaranteed unless confirmed.
8. Cancellation and Refunds : The designated period for cancellations is dependent on the course type as follows:

Scheduled Course:
Up to 7 days prior to the start of the course

Room rental/Closed class:


Up to 15 days prior to the start of the course

Client Site Course:


Up to 15 days prior to the start of the course
In the event of a cancellation by the customer, the following refunds will apply:

Notice received from the customer within the designated period: 50% refund

Notice received from the customer outside the designated period: 0% refund
All notifications from the customer must be received in writing or fax.
9. Payment: Unless note # 6 applies, full payment is requested on the first day or earlier. EduPro reserves the right to refuse the participation of any
confirmed participant due to this. Should previous arrangements be made for later payment, all student materials will be withheld until this is
complied with. Cancellations and late payment will forfeit most of our promo benefits.
10. Over Payment: There would be no refund nor credit should there be any excess amount paid, e.g. paying the VAT when your company is VAT
exempted. The same rule applies for advance payment for courses that still has to be confirmed. We recommend separate checks for separate
courses.
11. Billing: Clients will be charged based on the number of attendees on the Confirmation or actual number of participants whichever is higher. Unless
replaced, absent but registered clients will still be billed.
12. Submission: Please fax to us your reservation at the latest one week prior to the start of the training.
13. Revisions: Please revise, initialize and re-fax the same form. We will not modify entries for the client.
