I. ACKNOWLEDGMENT I would like to express my deepest appreciation who provided me to complete this portfolio. To our Professor Sheryl Morales who encourage, help, guide and give her more effort for us. I would also like to acknowledge my family, especially for my Mom who also help me to guide and give some advices. And last, thank you to our supervisors in Quezon City Health Department who also gave us opportunity to help them and to help their patients too.

II. INTRODUCTION This training helps me to improve my skills and knowledge. As a part of Quezon City Health Department, they allow us to be part of their company and we assigned in City Clinic where the dog and cat bite admit, also the check up for the employee of Quezon City Hall. Ive learned so much on this On-the-Job Training that also help on my career in the future.

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at City Clinic is they give more attention is the employees of City Hall and the dog and cat bite for anti-rabies. The strength and challenges of this company is that they are so demanding even the employees of City Hall. They want to served first before the outside zone patient even if includes of the policies in this clinic is the first come first served and they are give us a bad informed. I think the best practice of this company is to give more attention of the patients, person with disability and senior citizens. My over-all observation in City Clinic is they are so hard-working and they are loved of what are given work to them. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: Admitting patients Filling records Giving information to patients about Anti-Rabies What we do weekly are the following: Admitting patients Filling records What we do monthly are the following: ____________________________________________________________ During my whole training in City Clinic, I think the problem/s that I have encountered was the patient who didnt follow instructions or follow the rules and we hear a bad words from them because we didn't they served first. So whats the purpose of giving them number if they did not following rules.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: Weve learned how to use the electronic blood pressure monitor and the manual monitor. We also learned how to do the works of the nurses and how to record files of the patients. B. Equipment, tools, testing apparatus handled (softwares used included) We handled equipments like blood pressure monitor, thermometer, weighing scale and we also encode and uses the computer. C. Strong points versus weak points When we noticed by the patients and employees. When the patients are complaining and saying negative words about us even if its not our fault.
